Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient needs related to mobility, endurance, pain, and other medical conditions. Based on this, they develop treatment plans, perform interventions to improve patient well-being, and monitor progress.

• Evaluate patients' functional status and therapy needs; assist physicians in developing care plans.
• Monitor and report patient responses and condition changes to the healthcare team.
• Treat patients to relieve pain, restore function, and maintain maximum functioning according to standards of practice.
• Educate patients, families, and healthcare team members about care plans, limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy license in California
• Current CPR certification
• Valid driver's license, vehicle insurance, and reliable transportation
