Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and needs, assisting in developing the therapy plan of care in collaboration with the physician.
• Observe and report patient responses to treatment and any condition changes to the supervising nurse and/or physician.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice.
• Instruct and inform the healthcare team, patients, and families regarding the plan of care, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in California
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to reliable transportation
