Job summary

York Teaching Hospital NHS Foundation Trust is recruiting for a post within a Primary Care Network to provide a first-contact specialist mental health service across GP practices, offering assessment, signposting and short-term interventions in community settings.

You will work collaboratively with GP teams, secondary mental health services and voluntary sectors to improve access to specialist support and ensure timely care for patients and their carers.

Qualifications

• Qualified Occupational Therapist, Mental Health Nurse or Social Worker
• Experience working in primary care or community mental health settings is desirable
• Ability to work autonomously within GP Practices and PCN teams

Responsibilities

• Undertake initial assessments and signpost to appropriate services
• Provide short-term psycho-social interventions within GP surgeries and patients' homes
• Offer specialist advice to GPs and liaise with secondary mental health services
• Coordinate care pathways across primary and secondary care to support patients
