Job summary

Dorset County Hospital NHS Foundation Trust in the community seeks a Frailty Nurse to join the WECS team. The role involves housebound patients and home visits to complete baseline assessments and ongoing care planning.

The nurse will contribute to MDT discussions, support with data collection, and ensure accurate records on SystmOne with bespoke templates. Regular duties include home visits, triage, and ensuring notes are detailed.

Responsibilities

• Conduct baseline assessments for housebound patients using an agreed proforma on a structured template (continence, skin, nutrition, care needs, unmet needs).
• Perform home visits and follow-up visits.
• Make referrals to the wider MDT (including third sector) when appropriate.
• Contribute to MDT discussions to support patient care.
• Engage in comprehensive data collection.
• Maintain comprehensive record keeping on SystmOne, using bespoke templates.
• Ensure work emails and mobile are checked regularly and that contemporaneous notes are detailed.
• Attend MDT meetings if requested.
