Job summary

Meridian HealthCare in Ohio is seeking a physician to join our integrated primary care and behavioral health team. This full-time role emphasizes comprehensive patient care, prevention, and coordinated services within an FQHC setting.

You will diagnose, treat, and manage acute and chronic conditions, develop individualized care plans, order labs and imaging, and collaborate with behavioral health and pharmacy teams to support patient wellness.

Qualifications

• MD or DO degree from an accredited medical school.
• Current, unrestricted Ohio medical license or eligibility for licensure.
• Board certified or board eligible in applicable specialty.
• Current DEA registration or ability to obtain.

Responsibilities

• Provide comprehensive primary and preventive care.
• Diagnose and treat acute and chronic conditions.
• Develop and manage individualized treatment plans.
• Order and interpret labs, imaging, and diagnostic testing.
• Prescribe and manage medications.
• Coordinate referrals and specialty care.
• Collaborate with behavioral health, pharmacy, and other care teams.
• Educate patients on treatment, prevention, and wellness.
• Maintain accurate and timely medical documentation.
• Support FQHC quality and performance initiatives.
