Job Description

Assist patients, families, and caregivers with psychological, social, emotional, spiritual, and practical issues related to cancer diagnosis and treatment. Support patients throughout all phases of the disease continuum, including diagnosis, treatment, survivorship, and palliative care. Provide oncology social work services to clients and their families.

Responsibilities
  1. Assess new treatment patients for social, financial, and caregiver support needs; develop intervention plans.
  2. Counsel patients and families on adjusting to cancer diagnoses and overcoming barriers to treatment adherence and resource access.
  3. Collaborate with Financial Counselors to address insurance and financial challenges and create intervention strategies.
  4. Participate in weekly multidisciplinary meetings, updating the care team on psychosocial issues impacting treatment adherence.
  5. Work with providers to facilitate end-of-life discussions with patients.
Qualifications
  • MSW required.
  • Experience in oncology healthcare preferred.
  • Epic experience is a plus.
