Job summary

Qualifications

• At least 1 year of healthcare coordination experience in public health or healthcare settings.
• Experience in care coordination, health administration, or direct clinical care preferred.

Responsibilities

• Arrange and coordinate health services for children in care, including urgent and preventive care.
• Maintain complete, confidential medical records per policy, standards, licensing, and funding requirements.
• Communicate medical needs to case managers, clinicians, and program staff; schedule appointments.
• Coordinate transportation and interpretation/translation services for medical visits.
