Job summary

Adult & Child Health seeks a Foster Care Specialist to support youth and foster parents, lead treatment planning, and liaise among treatment teams to ensure safety, stability and goal achievement for permanency. The role emphasizes collaboration with families, DCS partners, clinicians, and community providers, with a focus on coordinated care and ethical practice.

Responsibilities

• Lead the treatment planning process upon youths' admission to a System foster home and develop the Treatment Plan with input from the team.
• Provide crisis intervention and emergency services as needed, including safety plans and reporting.
• Coordinate and support integrated care and maintain relationships with providers and stakeholders.
• Maintain timely, accurate documentation and meet reporting requirements.
• Attend required trainings and participate in supervision to ensure professional development.
