Centene is seeking a Foster Care Liaison hybrid role with about 25% local travel to DCFS/CWCA offices in northern Cook County. Responsibilities include care coordination, developing personalized care plans, and educating members and families on benefits and services. Training is M-F, 8-5, with main schedule 8-4:30/5 pm CT.

Preference to Cook County residents. Pay range is $22.94-$38.79 per hour. Remote/hybrid, field or office work options available, with comprehensive benefits and equal

Competenze

• Bachelor's degree in Social Work, Nursing, Health Behavioral Science or equivalent experience.
• 2+ years of experience working within a social service agency and/or advocacy environment.
• Must reside in Illinois.

Mansioni

• Evaluates member needs and barriers to care and facilitates the care plan.
• Develops or contributes to personalized care plans and coordinates providers or resources.
• Provides psychosocial and resource support to members and caregivers to access local services.
• Coordinates between member/family and care provider team to ensure timely access to services.
