CHS, serving Florida's children and families, invites you to apply for the Dependency Case Manager role. You will identify and assess the needs of abused/neglected minors in care by DCF, coordinate services, develop plans, and support permanency goals in collaboration with families and professionals.

Ideal candidates have a Bachelor’s degree, Florida Child Welfare Case Manager certification, and at least two years of relevant experience.

Qualifications

• Bachelor’s degree from an accredited university, required.
• Bachelor’s degree in a Human Services related field, preferred.
• Master’s degree in Social Work, preferred.
• Florida Child Welfare Case Manager certification, required.
• For Adoptions: Adoption Competent certificate or must acquire within 6 months of employment, required.

Responsibilities

• Identify and assess client and family needs of minors placed in care by DCF with the goal of permanency.
• Coordinate services, referrals and linkages to clients and families.
• Develop and maintain case and program documentation (assessments, treatment and service plans, progress notes, termination summaries).
• Prepare case reports and testimony for court or other permanency proceedings.
• Coordinate services with supervisor, team members, providers, DCF and attorneys as needed.
