Job summary

Cardinal McCloskey Community Services (CMCS) is seeking a dedicated Case Planner for its Foster Care Program in New York. You will advocate for children and families, guide them through transitions, and coordinate with foster parents, biological families, and service providers to ensure each child receives needed support.

Responsibilities include managing caseloads, developing individualized plans, supporting foster parents, and ensuring effective communication with schools and service

Qualifications

• BA / BSW and 2 years Human Services experience required.
• Work experience in Child Welfare &/or Office of Mental Health System preferred.
• Computer literacy required.
• NYS driver's license preferred.
• Bilingual in Spanish preferred.

Responsibilities

• Manage a caseload of children in foster care, ensuring their safety and well-being
• Develop and implement individualized treatment plans focused on permanency and stability
• Support foster parents with parenting strategies and behavioral guidance
• Maintain consistent communication with families, schools, and service providers
• Represent cases in Family Court and ensure every child's voice is heard
• Document progress and maintain compliance with agency and ACS standards
