Job summary

As an Unaccompanied Children (UC) Transitional Foster Care Case Manager, you will be responsible for providing casework and supportive services to children in foster care, foster families, and biological families referred to Refugee and Immigrant Services TFC program.

This position functions effectively with moderate supervision while following agency, federal, and state regulatory requirements.

ESSENTIAL JOB RESPONSIBILITIES

  1. Complete trauma-focused initial intake assessment within 24 hours of arrival and ensure youth feel safe and are well oriented to the agency and foster home.
  2. Coordinate contact of youth sponsors immediately upon arrival, ensuring the sponsor obtains all applicable documentation such as the Family Reunification Packet (FRP).
  3. Identify a sponsor by working with the consulate or equivalent resources to assign a sponsor for the youth, if unknown upon youth’s arrival.
  4. Complete the FRP using a trauma-focused lens in a timely manner to ensure youth’s safe release.
  5. Ensure all assigned youth receive mandatory services while in care, including mental health, medical, recreational, educational, and other supportive services, in accordance with program timelines.
  6. Maintain accurate and current information on foster children in care, ensuring compliance with ORR and Bethany’s documentation and reporting requirements.
  7. Lead the treatment team in conjunction with the youth’s therapist.
  8. Participate in weekly staff meetings with the treatment team and stakeholders.
  9. Provide ongoing education and support services to foster parents.
  10. Consult with supervisor on cases, review case plans, and evaluate client progress during weekly meetings.
  11. Co-lead education group sessions with educational staff.
  12. Assist licensing team to ensure foster families are well trained and equipped.
  13. Meet with foster parents and children monthly to address concerns.
  14. Translate written and verbal communication for program clients and staff, if bilingual.
  15. Communicate effectively with community agencies involved in service provision.
  16. Attend training sessions and apply new knowledge in service delivery.
  17. Prepare for and attend court hearings as applicable.
  18. Transport clients as needed.
  19. Refer families to community resources for support.
  20. Stay informed of all regulatory requirements related to social services.
  21. Perform other duties as assigned.

QUALIFICATIONS:

  1. Bachelor’s degree in Behavioral Sciences, Human Services, or a related field from an accredited college.
  2. Child welfare and/or case management experience preferred.
  3. Knowledge of social work principles and methodology.
  4. Bilingual in English and Spanish, with experience working with refugee and/or immigrant populations preferred.
  5. Mature judgment and cultural sensitivity.
  6. Excellent communication skills.
  7. Clinical, therapeutic, and crisis intervention skills.
  8. Ability to work independently and maintain confidentiality.
  9. Proficiency in Microsoft Office suite.
  10. Must be 21+, hold a valid driver’s license with at least 3 years of driving experience, pass MVR check, and maintain vehicle insurance.
  11. Pass criminal history and child protection registry checks.
  12. Immunization against vaccine-preventable diseases or willingness to be vaccinated.
  13. Alignment with the agency’s faith and mission statements.
