Job summary

FrontLine Service in Cleveland, OH is seeking a Community Transition Program (CTP) Case Manager to provide supportive services and case management to adults transitioning from an institution to homelessness, with a substance use diagnosis. You will complete progress notes and maintain documentation per agency standards.

This is a non-exempt, union position with a starting wage of $21.00 per hour. The hours are 1st shift, Monday through Friday.

Qualifications

• Bachelor’s degree in social, behavioral or human services required.
• At least one year of professionally supervised work in mental health, chemical dependency or social services preferred.

Responsibilities

• Provide supportive services and case management to adults discharged from an institution to homelessness with a substance use diagnosis.
• Complete and maintain progress notes and other pertinent documentation according to agency and regulatory service standards.

Education

Bachelor’s degree in social, behavioral or human services

CDCA/CCDC preferred
