The Commonwealth of Pennsylvania seeks a Forensic Social Worker 1 to support patients at Torrance State Hospital. You will provide social services and counseling to individuals ordered through the criminal justice system, working with families, staff, and external agencies to advance recovery and stability.

Responsibilities include interviewing patients, assessing needs, collaborating with a multidisciplinary team, documenting progress, and planning discharge or continued care.

Qualifications

• Bachelor's or master's degree in social work or social welfare.
• Within three months of graduating with qualifying degree.
• PA residency required.
• Able to perform essential job functions.
• Medical examination and drug screening required.
• Older Adult Protective Services Act compliance.
• Criminal history reports including PA State Police and FBI clearance.

Responsabilités

• Interview forensic patients, families, and agency contacts to gather social, medical, and emotional information.
• Assess patient needs and provide education and therapy to support improved functioning.
• Collaborate with multidisciplinary staff to develop and carry out individualized care and treatment plans.
• Document patient progress toward social service goals and communicate updates to involved parties.
• Prepare discharge or transfer plans with continued care or follow-up needs.
• Maintain current court commitment records and complete recommitment petitions.
