Job summary

Horizon Services, Inc., Palm Avenue Detox in California, seeks a Forensic Peer Specialist/SUD Counselor 1 to support clients as they enter treatment, provide relapse prevention coaching, and monitor vital signs as needed.

You will deliver group and individual sessions, help families understand recovery processes, document in the EHR, and collaborate with the Program Manager to ensure timely referrals and smooth discharge planning.

Qualifications

• Certification or registration with CA substance use counselor board.
• Experience with crisis intervention for high-risk clients.
• Strong communication and documentation skills.

Responsibilities

• Engage with client to set care plan goals for relapse prevention.
• Provide group and individual recovery coaching and empowerment.
• Document in EHR and track daily census; ensure accurate records.
