Job summary

Horizon Services, Inc. is recruiting a Forensic Peer Specialist to engage with program participants at the Diversion Triage Center in Pleasanton, CA. The role emphasizes monitoring, care, crisis intervention, and de-escalation within a collaborative team.

You will support clients with substance use challenges and co-occurring conditions, using compassionate care and evidence-based practices, while maintaining a safe environment and following daily schedules.

Qualifications

• 1 year of client monitoring experience with SUD/MH populations.
• Basic academic knowledge and understanding of Substance Use.
• Certified as a Forensic Peer Specialist or eligible to become certified.
• Preferred: bilingual in English, Spanish, Chinese, Vietnamese, or Tagalog.

Responsibilities

• Monitor and observe clients to ensure safety and accountability.
• Conduct property safety checks on incoming property.
• Monitor daily bed roster and census for program requirements.
• Assist with meal preparation and distribution.
• Collaborate with a multi-disciplinary team at the Triage Center.
• Communicate needs or concerns to Health/SUD staff.
• Participate in daily shift change discussions and address concerns.
• Create and maintain a safe, welcoming environment for clients.
• De-escalate emotionally unstable client behavior.
• Provide orientation for incoming clients.
• Attend to clients’ needs while following schedule.
• Maintain facility cleanliness and organization daily.
• Monitor cleanliness and perform cleanup duties as needed.
• Prepare bed rolls and basic hygienic items for new clients.
• Ensure laundry details are completed.
• Replenish supplies as directed by Program Manager.
• Provide assistance with transportation and emergency response.
• Transport clients to local facilities when needed.
• Respond to emergency needs as directed by Program Manager.
• Assist staff with referrals or transfers to continuing care.

JOB TITLE: Forensic Peer Specialist I-III

LOCATION: Diversion Triage Center - Pleasanton, CA

Employment Type: Full-time

Sign-On Bonus for Bilingual Applicants: $2,000 (Distributed in 4 parts: $500 after 30 days, $500 after 90 days, $500 after 150 days, and $500 after 210 days of employment)

The bilingual bonus applies to client-facing positions and will be awarded upon successful completion of the language test.

The Forensic Peer Specialist position is a key position in carrying out daily activities and establishing connection with program participants at the Triage Center (Pleasanton and/or San Leandro). The position is central to monitoring and maintaining organization, structure and safety for all staff, clients and community members arriving and or residing at the Triage Center.

We are seeking a Forensic Peer Specialist who is passionate about serving diverse communities. Bilingual candidates fluent in BHSD’s Threshold language— Spanish are eligible for an additional incentive.

In this role, you’ll make a difference in the lives of people with substance abuse challenges, and often with co-occurring conditions. You’ll use and grow your skills in compassionate care, crisis intervention and de-escalation, in a supportive and inclusive work environment. You’ll have the opportunity to provide our community with substance abuse treatment to help people who are just discovering possibilities in their journey, going from a place of hopelessness to a place where they are valued and have the potential for a better life.

Responsibilities

QUALIFICATIONS: (education and work experience)

• 1 year of client monitoring experience working directly with SUD/MH populations
• Basic academic knowledge and understanding of Substance Use
• Demonstrated competency in using technology
• Certified as a Forensic Peer Specialist or eligible to become certified. Horizon Will Help with
