We're seeking an experienced Case Management Assistant II to support a busy Care Coordination team within a leading healthcare organization. This is a non-clinical position focused on discharge planning, transition-of-care coordination, and skilled nursing facility admissions and intake.
This is a Contract position based out of Mather, CA.
The pay range for this position is $24.00 - $24.00/hr. Individual compensation offered within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
