Job summary

Impact Life in Dover, Delaware is seeking a dual-certified FNP/PMHNP to provide primary and behavioral healthcare within our Rural Health Hub and Integrated Outpatient Services. This role focuses on low-barrier, whole-person care for individuals with substance use disorders and complex social needs.

You will assess medical and behavioral health needs, prescribe MOUD and psychotropic medications, and collaborate with an interdisciplinary team to expand access to recovery-oriented care in the

Qualifications

• Current, unrestricted Nurse Practitioner license in Delaware.
• Dual FNP/PMHNP certification required.
• Board certification through ANCC or AANP.
• Active DEA registration and prescriptive authority.
• Ability to prescribe MOUD per federal/state rules.
• Minimum 2 years of clinical experience in primary/behavioral/integrated care.
• Strong understanding of integrated care models and social determinants of health.

Responsibilities

• Conduct comprehensive medical and behavioral health assessments.
• Diagnose and treat acute and chronic conditions within scope.
• Provide MOUD and psychotropic medication management.
• Develop person-centered treatment and care plans.
• Coordinate care with interdisciplinary team.
• Maintain accurate EHR documentation.
• Participate in quality improvement activities.
