Job summary

Northeast Healthcare Recruitment is seeking a Family Practice Family Nurse Practitioner in Hyannis, MA to provide comprehensive primary care. The role includes routine visits, chronic disease management, acute illness care, and care coordination with the patient panel.

The position is full-time with a competitive salary starting at $124,800 plus experience-based pay, plus benefits such as health, dental, vision, life and disability insurance, CME stipend up to $2,500 and paid time off.

Qualifications

• Completed accredited Nurse Practitioner program in Family Practice.
• Current Family Practitioner license and board certification.
• Proficient in routine health maintenance and chronic disease management.

Responsibilities

• Serves as a primary care provider for a panel of patients.
• Performs interval well visits including history, exam, cancer screening, and health promotion.
• Participates in staff meetings, trainings, and quality improvement sessions.

Job description

FNPprovides care appropriate to the patient population served, including routine healthmaintenance, management of chronic conditions, and management of acute illnessfor patients. Position is full time

Responsibilities:

• Serves as a primary care providerfor a panel of patients.
• Provides routine healthmaintenance by performing interval well visits. This may include acomplete history and physical, developmental assessment, risk assessment,cancer screening, and health promotion education. Follows up on anyproblems detected. Makes appropriate referrals for additionalservices required, including nutrition, social services, behavioralhealth, specialty care.
• Participates in provider staffmeetings, departmental and general staff meetings, trainings, care teammeetings and quality improvement/practice transformation sessions

• Completed accredited NursePractitioner program in Family Practice
• Current License, FamilyPractitioner, Board Certified

Minimum salary is $ 124,800 plus dependent on experience
