Description

Clinical Staff Pharmacists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments, they provide support of centralized and decentralized medication use systems and deliver optimal medication therapy to patients with a broad range of disease states. All pharmacists are responsible for drug preparation and distribution for medications. They evaluate, input, and verify medication orders. They assist providers and other healthcare professionals by serving as a drug information resource. Pharmacists monitor and manage medication regimens per protocol or per consult. They manage pharmacy inventory. Pharmacists have a working knowledge of pharmacy operations including pharmacy operating systems, use of patient records, purchasing, inventory, and formulary management. They are involved in automation, packaging, and pharmaceutical waste management. Pharmacists initiate, document, and participate in activities that promote positive patient outcomes. These activities may include identifying medication process issues and implementing solutions. They engage in clinical and cost savings initiatives, therapeutic drug monitoring, and appropriate formulary utilization. Participation in emergency response activities may also be required. They conduct medication usage evaluations and adverse event reporting. Pharmacists work with inter-professional teams to optimize order sets, inventory, and medication-related policies. They serve as drug information experts regarding medication management. Pharmacists provide comprehensive clinical services including anticoagulation management, pharmacokinetic dosing, renal dosing, and nutritional support services. Participation in antimicrobial stewardship and opioid stewardship is expected. They provide medication counseling to patients. Pharmacists work with inter-professional patient care teams to assist patients with transitions in care. They also help prepare patients for discharge. Pharmacists serve as active members of patient care teams and participate in unit-based huddles and activities if available. They are knowledgeable in unit metrics and goals, collaborating with staff to help achieve these goals. Pharmacists interpret and review medication orders for accuracy regarding drug, dosage, route, frequency, allergies, and potential adverse events for all ages served. They ensure the timely availability of medications to avoid delays in therapy. Pharmacists oversee the dispensing and reconciliation of medications, including controlled substances, and manage expiration dates and drug recalls. They perform medication reconciliation for patients. Pharmacists provide technician oversight and assume supervisory roles in daily activities. They supervise and perform the production of compounded products for first doses and scheduled deliveries. Pharmacists demonstrate the necessary supervisory skills to plan, organize, and direct pharmacy activities in a cost-effective manner. They assist in the orientation and training of pharmacists, technicians, students, and residents. Pharmacists act as preceptors for students and residents as applicable. They maintain knowledge of local, state, and federal regulations regarding pharmacy practice. Pharmacists effectively utilize pharmaceutical references and promote formulary therapies to medical staff. They contact providers with alternatives and use their knowledge of medication formulary and non-formulary processes to make appropriate recommendations.
