Job summary

Sarasota Memorial Health Care System seeks a Clinical Care Coordinator II to provide patient care aligned with age-specific needs in the physician office setting under direct supervision of a provider.

Position is PRN LPN with a pay rate of $25/hr. Travel to FPG offices across Sarasota, Venice, and Lakewood Ranch; eight-hour shifts; self-scheduling; availability of at least 4 days per month; offices open 8am-5pm, Monday-Friday.

Qualifications

• Requires Basic Life Support/CPR certification, or must be obtained within 30 days of hire/transfer date.
• Graduate of an accredited school of nursing or a diploma program.

Job Summary

The Clinical Care Coordinator II demonstrate the competency to provide patient care according to the age-specific population needs and assist in the management of patient care in the physician office setting under the direct supervision of a provider.

Pay rate is set at $25/hr for PRN LPN position.

Required Qualifications

• Require Basic Life Support/CPR certification; or must be obtained within thirty (30) days of hire/transfer date.
• Require graduate of an accredited school of nursing or a diploma program.

Preferred Qualifications

• Prefer one (1) year of clinical experience in outpatient physician office practice.
• Prefer experience with an electronic practice management and electronic health record.
• Prefer demonstrated knowledge of electronic practice management and electronic health records software along with skill in all Microsoft office software programs (Word, Excel, Outlook, and Power Point).

FL LPN: FL Licensed Practical Nurse

Work Days/Shift/Start Time

Candidated will travel to FPG offices (located in various areas including Sarasota, Venice, Lakewood Ranch); Self-scheduling available; Need to be available minimum 4 days per month.

8 hour shifts - most offices are open between 8 AM and 5 PM, Monday-Friday.
