Job summary

SSM Health Rehabilitation Hospital is seeking a Case Manager - Float to coordinate care across three Missouri locations: Bridgeton, Lake St. Louis, and Richmond Heights.

The role is full-time, with 8-hour days, and involves discharge planning, utilization management, and coordinating treatment plans to improve patient outcomes. The position offers a salary range of $77,000 to $105,000 based on experience, with opportunities for professional growth within a large health system.

Qualifications

• Current licensure per state guidelines in clinical or related discipline (RN preferred) OR a Bachelor's or Master's in a health or human services discipline.
• CCM certification preferred.

Responsibilities

• Coordinate health care decisions to improve quality and outcomes.
• Coordinate care across the continuum to promote seamless transitions.
• Promote cost-effective care within the allotted time frame.
• Assure payments of hospital-based services meeting patient-related utilization management criteria.
• Implement safe and appropriate discharge plans and assess psychosocial needs as part of discharge planning.
