SSM Health Rehabilitation Hospital is seeking a Case Manager - Float to coordinate care across Bridgeton, Lake St. Louis, and Richmond Heights locations. This full-time role involves discharge planning, care coordination, and payer management to ensure high-quality outcomes.

Qualifications

• Current licensure per state guidelines in clinical or related discipline (RN preferred) or a Bachelor’s or Master’s degree in a health or human services discipline.
• CCM Certification is preferred.

• Coordinate health care decisions to improve quality and outcomes across the continuum.
• Plan and implement discharge plans ensuring safe and appropriate care.
• Manage finances and payer/referral aspects of case management.
