Sequoia Care is seeking a Float Physical Therapy Assistant (PTA) to provide flexible clinical coverage across multiple outpatient therapy clinics located within assisted living and senior living communities. The Float PTA serves as a regional resource to maintain continuity of care during vacations, vacancies, and leaves while supporting expansion of therapy services.

Qualifications

• Graduate of an accredited Physical Therapy Assistant Program.
• Current licensure as a Physical Therapy Assistant in the practicing state.
• Additional certifications or registry as required.
• A strong understanding of the needs of patients, colleagues, and the broader community.

Responsabilités

• Evaluate and treat outpatient orthopedic, neurological, and geriatric patients.
• Develop individualized treatment plans.
• Perform re-evaluations and discharge planning.
• Document services accurately and timely within EMR.
• Coordinate care with physicians, nursing staff, and caregivers.
• Educate patients and families on home exercise programs and fall prevention.
• Provide temporary coverage for PTO and vacation relief, medical leaves, open positions, census fluctuations, new clinic launches, and adapt workflows.

Formation

Graduate of an accredited Physical Therapy Assistant Program
