Job summary

Hartford HealthCare is seeking a Float Primary Care Nurse (RN) to deliver skilled nursing care in home and community settings. This role emphasizes patient-centered, collaborative care and requires a CT-licensed RN with at least one year of nursing experience.

Qualifications

• Graduate of an Associate’s Degree or diploma program in nursing.
• One year nursing experience or one year nursing experience in a specialty.
• Licensed registered nurse in CT.

Responsibilities

• Maintain a client record utilizing the Agency’s standard for documentation.
• Make follow-up visits to provide skilled care and assess disease progress.
• Evaluate response to treatment and provide support to patients/families.
• Teach patients and reinforce client/caregiver education.
• Coordinate care with providers and consult with physicians regarding client needs.
• Communicate with Nurse Care Coordinator and all involved in the patient’s care.
• Direct LPNs in carrying out the established plan of care.
• Supervise Home Care Aides in executing the plan of care.
• Refer clients/families to community resources as needed.
• Attend and participate in in-service education programs.
