Job summary

Hartford HealthCare is seeking a Primary Care Nurse (RN) in Southington, CT to deliver skilled nursing care in home and community settings. You will perform visits, coordinate with physicians, supervise LPNs and aides, educate patients, and document care according to agency standards.

This full-time role requires CT RN licensure and a patient-focused approach. Ideal candidates have an Associate Degree in Nursing, at least one year of nursing experience, and a preference for NLN-approved BSN

Qualifications

• Graduate of an Associate’s Degree or diploma program in nursing.

Responsibilities

• Maintain client records per documentation standards.
• Make follow-up visits to provide skilled nursing care.
• Evaluate treatment responses and support patients and families.
• Educate patients and caregivers.
• Coordinate care with providers and consult physicians.
• Communicate with Nurse Care Coordinator and care team.
• Direct LPNs in carrying out the plan of care.
• Supervise Home Care Aides implementing the plan of care.
• Refer clients to community resources as needed.
• Attend in-service education programs.
