Job summary

Yakima Valley Farm Workers Clinic is seeking a Float Pharmacist to support pharmacies across multiple locations, including Salem, Woodburn, and Portland. The role involves direct patient care, medication management, and collaboration with clinicians within a team-based setting.

Qualifications

• RequiresGraduate in Pharmacy with active license (Washington).
• PharmD degree and bilingual English/Spanish preferred.

Responsibilities

• Provide patient care services including Drug Utilization Review, Patient Counseling, and Medication Therapy Management.
• Possess authority to prescribe or modify care plans under collaborative agreements.
• Order, interpret, and monitor lab tests; ensure safe dispensing per policies and legal standards.
• Oversee pharmacy staff and maintain security and compliance with records and inventories.
• Maintain licensure and pursue ongoing professional education.
