Job summary

Catholic Charities Family and Community Services (CCFCS) in New York is seeking a Float LPN to provide medication management across residential settings under nursing supervision. You will document medications in EMAR and support client wellness within the residence.

The role requires an NYS LPN license, a high school diploma or GED, and 2–3 years of nursing experience in MH/chemical dependency. Bilingual Spanish is preferred.

Qualifications

• High School Diploma or GED and NYS LPN diploma required.
• Valid NYS Licensed Practical Nurse license.
• Bi-lingual in Spanish preferred.
• Two to three years of nursing experience in mental health and chemical dependency services preferred.

Responsibilities

• Provides medication management within the residential setting.
• Oversees residential self-medication per prescriptions.
• Documents medication self-administration in EMAR for each client.
• Procures and logs medications for each client.
• Monitors client medication adherence and communicates with prescribers and medical staff.
• Communicates with residential staff and takes part in patient discharge planning.
• Supports client wellness and coordinates with medical teams to ensure appropriate care.
