Qualifications

• Conduct quarterly in-person visits and maintain monthly contact with members by phone.
• Arrange non-medical supports (social integration, housing, employment, community resources).
• Ensure cost-effective service delivery and evaluate risk factors.
• Maintain accurate documentation including case notes, service authorizations, and MCP updates.
• Travel to visit members in their setting approximately 40% of the time.

In this dynamic role, you will collaborate with dedicated colleagues to deliver care and empower our members to excel in their daily lives. You will be a \"float\" associate and cover caseloads for team members in the SW Pod 2 region (Trempealeau, Jackson, La Crosse, Monroe, and Clark counties) that have exited their role or those on a leave.
