Bluestone Physician Services seeks a Float Care Coordinator to support DHS and CMS activities for Minnesota Senior Health Options (MSHO/MSC+) in community settings, skilled nursing and assisted living facilities. This full-time role is remote with regular local travel; about 50–70% travel in the seven-county metro area and 30–50% remote work.
Hours: 8am–5pm Mon–Thu and 8am–3pm Fri. Qualifications include MN Licensed Social Worker or MN RN license, valid driver’s license, and experience with

Our care teams travel directly to patients residing in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida, supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Join our team as a Float Care Coordinator, where you will work with primarily senior populations, managing Department of Human Services (DHS) and Centers for Medicare/Medicaid Services (CMS) required activities for Minnesota Senior Health Options (MSHO/MSC+), and potentially SNBC (ages 18-64) members, in community settings, skilled nursing and assisted living facilities. This position is remote with regular local travel to serve members and the community.
Schedule: Full-time, day shift hours, no evenings, weekends or holidays. Hours are 8am–5pm Monday through Thursday and 8am–3pm on Fridays.
Location: ~50-70% travel throughout the seven-county metro area, and ~30-50% remote work.
Salary Range: $65,000 - $75,000. Salary commensurate with experience.
