Qualifications

• Compassionate, dependable caregiving required or experience in home health.
• Strong communication skills and professionalism in interactions.
• Must be punctual, reliable and able to follow care plans.

Responsibilities

• Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and personal hygiene.
• Provide companionship and emotional support.
• Assist with mobility, transfers, and fall prevention.
• Prepare meals and assist with feeding when needed.
• Perform light housekeeping, laundry, and household tasks.
• Provide medication reminders as allowed by policy.
• Assist with errands, grocery shopping, and transportation when authorized.
• Observe and report changes in clients’ conditions.
• Maintain documentation and communicate with the care team.
