You will conduct PT evaluations, design individualized treatment plans, and help residents improve mobility and independence in a skilled nursing setting, collaborating with an interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapist license or eligibility for licensure prior to employment.
• Strong clinical reasoning, communication, and interpersonal skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to providing individualized, high-quality rehabilitation services.
• Experience in skilled nursing, geriatrics, or post-acute rehabilitation is a plus, but not required.
• Experienced Physical Therapists and New Graduates Are Welcome to Apply!

Responsibilities

• Conduct comprehensive physical therapy evaluations to assess mobility, strength, balance, functional limitations, and rehabilitation needs.
• Develop individualized, evidence-based treatment plans based on residents' clinical needs and goals.
• Provide therapeutic exercises, gait training, balance activities, transfer training, and functional mobility interventions.
• Help residents improve mobility, restore function, reduce fall risk, and achieve greater independence.
• Collaborate with occupational therapy, speech therapy, nursing, and other interdisciplinary team members.
• Educate residents and families on safe mobility, therapeutic exercises, and injury prevention.
• Maintain accurate and timely clinical documentation in accordance with professional and regulatory standards.
• Participate in care planning and discharge recommendations as appropriate.
• Support a positive, patient-centered rehabilitation environment.
