Job summary

Delta-T Group is seeking Licensed Practical Nurses to provide service for Group Home facilities in Farmington, CT and surrounding areas. All shifts and days are available, with compensation of $36+ per hour.

As part of the role you will administer and monitor residents' medical needs, maintain thorough medical records, collaborate with prescribing physicians and other health care providers, build professional rapport with residents, and ensure facility cleanliness, safety and health.

Qualifications

• CT LPN certification
• Current CPR certification
• Background check will be required including fingerprinting

Responsibilities

• Administer and monitor residents' medical needs in accordance with orders by prescribing physicians
• Maintain ongoing relationships with other health care providers to provide consistency of prescribed treatment procedures and techniques
• Maintain medical records and oversee thorough documentation
• Develop professional relationships and rapport with facility residents
• Ensure facility cleanliness, safety, and health
