Job summary

Avenues Home Care is hiring a Home Health Aide - HHA to support elderly and vulnerable clients in the local area, including Brentwood, Nashville, and Murfreesboro. You will assist with ADLs, medications, meals, transportation and companionship, while reporting activities through the agency system.

Ideal candidates have good communication, follow verbal and written instructions, and possess or are pursuing CNA experience. CPR/First Aid are preferred and reliable transportation is required.

Qualifications

• Good communication skills required.
• Ability to follow verbal and written assignments.
• Proficiency with ClearCare management system.
• 18+ years old; valid driver’s license and auto insurance.
• CNA license or equivalent caregiver experience preferred.
• CPR and First Aid certifications preferred.
• Reliable transportation.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) as per care plan.
• Assist with medication, housekeeping, meals, shopping, transportation and companionship.
• Report daily activity to supervisor via agency management system and logs.
• Maintain client safety and confidentiality per company policies and HIPAA.
