Job summary

Medi Home Health Agency, a division of Medical Services of America, Inc., is seeking a Full-Time Physical Therapist to join our home health team in the South Hills (Allegheny County), PA. You will provide in-home physical therapy, follow care plans, and collaborate with families and physicians to improve mobility and independence.

Qualifications

• Active Physical Therapy license in PA required.
• Current CPR certification required.
• Valid driver’s license and reliable transportation.
• At least 1 year of physical therapy experience (home health preferred).

Responsibilities

• Provide in-home physical therapy services across assigned territory.
• Follow individualized care plans and adjust treatment as needed.
• Help patients reduce pain, improve mobility and regain independence.
• Collaborate with families, physicians, and care team members.
• Monitor and document progress to ensure goals are met.
