Job summary

Horizon Home Health Care in Montgomery County, Ohio is seeking a licensed Speech-Language Pathologist to provide in-home therapy on a part-time/PRN basis. You will evaluate and treat speech, language, cognitive, voice, and swallowing disorders, delivering one-on-one care in patients' homes.

This role offers flexible scheduling, weekly pay, per-visit compensation, and a supportive clinical team, with emphasis on patient-centered outcomes and collaboration with physicians and other providers.

Qualifications

• Current Ohio SLP license.
• CPR and/or BLS certification.
• Valid driver's license with reliable transportation and auto insurance.
• Excellent communication and organizational skills.
• Home health experience is preferred but not required.

Responsibilities

• Evaluate and treat speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on patient goals.
• Educate patients and caregivers on treatment strategies and home programs.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians, nurses, therapists and other disciplines of the healthcare team.
• Maintain compliance with state and federal home health regulations.
