Job summary

Professional Case Management (PCM) is hiring RN clinicians to provide in-home care in Jackson, SC on a part-time or full-time basis. You will create one-on-one care plans and deliver nursing services in clients' homes under the state Nurse Practice Act and agency policies.

Qualifications

• Graduate of a state-approved school of professional nursing (BSN preferred).
• Current, unrestricted RN license in the state(s) of practice.
• Current CPR certification and TB test.

Responsibilities

• Assess and evaluate health status of assigned clients.
• Review plan of care to evaluate client response to interventions.
• Verify medical orders and administer prescribed medications.
• Maintain a safe and effective home environment for clients and coworkers.
• Comply with agency policies and professional standards.
• Communicate observations and nursing documentation clearly.
