Job summary
Concord Health Systems in Western Kentucky is seeking dependable LPNs and RNs for multiple shifts across our locally owned facilities. We put residents first and provide nurse support to ensure compassionate, high-quality care in a skilled nursing and long-term care environment.

Qualifications
- Current LPN or RN license in Kentucky.
- Experience in long-term care, skilled nursing, or nursing home preferred.
- Strong clinical, organizational, and communication skills.
- Reliable attendance and flexibility to work assigned shifts.
- Commitment to compassionate, professional resident care.
Responsibilities
- Administer medications and treatments in accordance with physician orders and facility policy
- Assess (RN) or monitor (LPN) resident conditions and report changes appropriately
- Provide direct nursing care in a skilled nursing and long-term care setting
- Accurately document care and treatments in the medical record system (EMAR/EHR)
- Collaborate with CNAs and interdisciplinary team members to deliver quality care
- Assist with care planning and follow established care plans
- Maintain compliance with state and federal regulations
- Contribute to a respectful, resident-centered environment
