Job summary

Home Helpers Home Care is seeking a qualified Registered Nurse (RN) to perform in-home client assessments on an as-needed, per-assessment basis. This role offers flexible scheduling and compensation per completed assessment, with no guaranteed weekly hours.

The RN will conduct comprehensive assessments, evaluate care needs, document findings, and communicate recommendations to the office, while maintaining professionalism and patient confidentiality in clients' homes.

Qualifications

• Current and valid Registered Nurse (RN) license.
• Experience with home care, geriatric care, assessments, or case management is preferred.
• Strong communication and organizational skills.
• Ability to work independently and manage assessment documentation accurately.
• Reliable transportation and willingness to travel to client homes as needed.

Responsibilities

• Conduct comprehensive in-home assessments for prospective and/or current clients.
• Evaluate the client’s care needs and overall condition.
• Gather and document relevant client information accurately.
• Complete all required assessment paperwork and documentation.
• Communicate assessment findings and recommendations to the office.
• Maintain professionalism and confidentiality when interacting with clients and families.
• Complete assessments within the required timeframe.
