Symbii Home Health and Hospice is looking for a Registered Nurse (RN) Case Manager to join our team in Idaho Falls. This role offers flexible hours, minimal weekends, and the opportunity for direct one-on-one patient care.

Qualifications

• Registered Nurse with a current RN license in Idaho.
• Experience in home health care preferred.
• Strong communication and patient education skills.

Responsibilities

• Develop and coordinate care plans with the interdisciplinary team.
• Provide counseling and education for patients and caregivers.
• Prepare clinical notes and manage discharge planning.
