Qualifications

Education: Associate degree in nursing; BSN preferred.
Experience: 1 year related experience or training.
Credentials: AHA BLS - Healthcare Provider, RN - NJ; ATCN/TNCC within 1 year if applicable; NIH Stroke Scale certification.

Responsibilities

Plans and provides individualized comfort measures; supports patient and family education.
Assists with evaluating interventions and outcomes; collaborates with case managers.
Documents discharge plans and coordinates care with treatment teams.
Communicates patient condition and treatment options to patients and families.
Manages information to tailor teaching for cultural diversity and patient needs.
Documents changes in clinical status and ensures effective handoff communications.
