Job summary

The Lakes Home Care, Inc. is seeking a dedicated Physical Therapist Assistant to join our team, delivering compassionate in-home rehabilitation under supervision of licensed therapists.

You will assist in developing and implementing personalized treatment plans for patients, promoting recovery and pain management in a supportive home-care setting. Ideal candidates have experience in geriatrics, orthopedics, or hospital environments and familiarity with inpatient therapy procedures.

Qualifications

• Previous experience in patient care, particularly in geriatrics, orthopedics, or hospital settings, is preferred or home care
• Knowledge of physiology and occupational health practices
• Familiarity with inpatient therapy procedures

Responsibilities

• Provide assistance to Physical Therapists in developing and implementing treatment plans
• Work with patients to help them regain movement and manage pain
• Monitor and document patient progress during therapy sessions
• Educate patients and their families on at-home exercises and care routines
• Collaborate with healthcare professionals to ensure comprehensive patient care
