Senior Care Solutions is seeking a compassionate Speech Therapist (ST) to deliver high-quality, patient-centered services in the home health setting on a PRN/Part Time basis in Boca Raton, FL. You will assess and treat communication, swallowing, and cognitive-linguistic needs one-on-one to help patients regain function at home.

Qualifications

• Active Florida Speech Therapist License.
• 1+ year of home health experience.
• Strong clinical assessment skills.
• Excellent communication abilities.
• Reliable transportation and valid driver\u2019s license.
• Ability to work independently on a PRN caseload.

Responsabilités

• Conduct comprehensive speech, language, cognitive-linguistic, and swallowing evaluations.
• Develop individualized treatment plans based on evaluation findings and patient goals.
• Provide skilled therapy interventions, including communication therapy, swallowing therapy, cognitive retraining, and compensatory strategy training.
• Educate patients and caregivers on home exercises, safety, and communication strategies.
• Document all care in compliance with Medicare and agency standards.
• Communicate regularly with nurses, therapists, and other interdisciplinary team members to ensure coordinated care.
• Report any significant changes in patient condition to the physician and clinical team.
