Job summary

Concept Rehab is seeking a Speech-Language Pathologist (SLP) on a PRN basis to join our therapy team in Waterford, MI. This flexible role supports a meaningful patient care mission within a therapist-owned organization.

You will evaluate, treat and document across communication, cognition and swallowing needs while collaborating with an interdisciplinary team to maximize patient independence and quality of life.

Qualifications

• Master's degree in Speech-Language Pathology.
• Licensed or eligible for Michigan SLP license.
• ASHA certification or eligibility.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled therapy for speech, language, cognition and swallowing.
• Monitor progress and modify treatment as needed.
• Complete timely clinical documentation.
• Communicate with patients, families and interdisciplinary team.
• Engage in care planning and discharge planning.
• Educate patients, caregivers and facility staff.
• Provide in-service training and support maintenance programs.
• Help patients maximize independence and quality of life.
