Concept Rehab in Bay City, MI is seeking a licensed Speech-Language Pathologist for a flexible PRN role in a skilled nursing and rehabilitation setting.

You will evaluate and treat residents with communication and swallowing needs, develop personalized plans, and collaborate with an interdisciplinary team to support safety and independence.

Competenze

• Licensed SLP in Michigan with active license.
• Master’s degree in Speech-Language Pathology from an accredited program.
• CCC-SLP preferred; CF candidates considered.
• Experience in skilled nursing, rehabilitation, or geriatric care beneficial.
• Strong assessment, organization, and communication skills.
• Ability to work independently and collaborate with interdisciplinary team.

Mansioni

• Complete speech, language, cognitive-communication, and swallowing evaluations as indicated.
• Develop individualized treatment plans based on residents’ clinical needs and functional goals.
• Provide therapy to address communication difficulties, cognitive deficits, and dysphagia.
• Recommend appropriate swallowing strategies and diet modifications in collaboration with the interdisciplinary team.
• Monitor resident progress and adjust interventions based on response to treatment.
• Maintain timely, accurate clinical documentation in accordance with facility and regulatory requirements.
• Communicate with nursing staff, physicians, families, and other rehabilitation professionals.
• Support resident safety, participation, and independence through evidence-based treatment.
