Nest & Care seeks a dedicated PRN Registered Nurse (RN) to join our home care team. You will provide essential nursing care to patients in their homes, offering flexible scheduling and meaningful impact. This role supports care plans, assessments, and collaboration with care managers.

Qualified candidates hold an MD RN license with Trach care experience, CPR/First Aid certification, and a willingness to travel for client visits. Prior home care experience is preferred.

Qualifications

• MD RN license is required.
• CPR and First Aid certifications are required.
• Two professional healthcare references preferred.

Responsabilités

• Meets new and prospective clients for an initial assessment (virtual or in-person).
• Develops a Plan of Care for all new clients and updates as needed.
• Communicates with care managers, caregivers, office staff, clients and families about needs.
• Consults with Clinical Director regarding supervision of services.
• Provides training to clients regarding their care when requested.
• Maintains clinical records and documentation per State and Federal laws.
• Performs client evaluations every 30, 90 or 120 days as needed by the Plan of Care.
• Performs caregiver evaluations as required by state laws.
• Trains new staff and existing staff.
• Provides Clinical Oversight.
