The role involves screening and treating patients with cognitive and ADL disorders, delivering skilled therapy, and documenting progress with adherence to regulatory standards. You will work within a collaborative team, follow CAPLICO values, and contribute to discharge planning while supporting families and caregivers.

Qualificações

• Licensed Physical Therapist Assistant credential is required.
• Open to applicants of all experience levels.

Responsabilidades

• Screen and evaluate patients with cognitive and/or ADL disorders and develop plans of care.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative treatment per regulatory requirements.
• Perform ADL retraining, therapeutic activities, exercises, modalities, and use of assistive devices.
• Assess progress and adjust treatments in the medical record per requirements.
• Evaluate outcomes and modify services based on evaluations and referrals.
• Offer counseling to patients, families, and other providers about physical disorders.
• Communicate progress and plans with supervisor and health team members.
• Provide discharge summaries of services delivered per regulations.
