Job summary

Innovate Rehab and Wellness in Louisiana's skilled nursing facilities seeks a Licensed PRN Certified Occupational Therapy Assistant to support residents in regaining independence and improving quality of life. You will provide therapy under OT supervision and deliver direct patient care.

This role emphasizes one-on-one treatment, real-world rehab outcomes, HIPAA-compliant documentation, and collaboration with nursing, PT, SLP. Flexible as-needed shifts support work–life balance.

Qualifications

• OTA degree and NBCOT certification required.
• Active state OTA license required.
• Experience in SNF or long-term care preferred.
• Ability to provide patient-centered care.

Responsibilities

• Deliver OT services per plan of care.
• Document resident progress in EMR.
• Assist with discharge planning.
• Collaborate with nursing and therapy teams.
• Ensure HIPAA compliance.
