Job summary

Blue Ash Health & Rehab, LLC in Blue Ash, Ohio is seeking a dependable PRN Occupational Therapist (OTR) to join our skilled nursing facility team. The role aims to help residents regain independence, daily living skills, and maintain quality of life on a flexible as-needed schedule.

The Occupational Therapist will evaluate residents, create individualized treatment plans, and work with an interdisciplinary team to support residents while adhering to regulatory standards.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Ohio Occupational Therapist license in good standing.
• Experience in a skilled nursing facility or long‑term care setting preferred.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans based on their needs and goals.
• Provide therapy services to help residents improve strength, coordination, mobility, and independence with daily activities.
• Assist residents with activities such as dressing, bathing, grooming, and other activities of daily living.
• Monitor resident progress and adjust treatment plans as needed.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Work closely with nursing staff, physical therapists, speech therapists, and other members of the interdisciplinary team.
• Participate in care planning and communicate resident progress and concerns to the appropriate team members.
• Follow all applicable safety standards, professional guidelines, and state and federal regulations.
