Job summary

Continuum Therapy Partners, LLC is seeking a licensed Occupational Therapist for a PRN daytime role at our Lake Country Health Services & Lake Country Landing locations in Oconomowoc, WI. You will evaluate patients and develop individualized treatment plans to improve functional independence, while documenting progress and adjusting plans based on outcomes.

Applicants must hold a Master’s or Doctorate in OT, current WI OT licensure, NBCOT certification, and prefer at least 1 year of experience.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy is required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
